pharmacist.

Then I decided to go back to school and become a physician.

It meant taking the MCAT, applying to medical school, giving up years of pharmacist income, going back into the classroom, taking board exams, spending countless hours in the hospital, and essentially starting over professionally.

And now, as a fourth-year medical student (as of 2026-2027) approaching the end of that journey, I realize something:

There are a lot of pharmacists who have quietly wondered whether they should do the same thing.

Honestly since I’ve started sharing my story on social media, I’ve had tons of pharmacists reaching out to me to ask me questions…

Maybe that’s you.

Maybe you love pharmacy but want a larger role in diagnosing and managing patients.

Maybe you’ve thought about medical school but can’t imagine giving up your pharmacist salary.

Maybe you’re wondering whether your pharmacy background would actually help you in medical school.

Maybe you’re trying to figure out whether becoming a physician would be worth the years of training, debt, sacrifice, and opportunity cost.

Those are exactly the questions I want to help you answer.

Not because I have everything figured out.

But because I’m actually living it.

And I’m documenting what I learn along the way.
<p class="button-wrapper" data-attrs="{"url":"https://michaelcogbonna.substack.com/subscribe?","text":"Subscribe now","action":null,"class":null}" to MD Scribe For?

The PharmD to MD Scribe is primarily for three types of people:

💊 Pharmacists Considering Medical School

Maybe the thought has crossed your mind once.

Maybe you’ve been thinking about it for years.

Either way, I’ll share what the transition actually looks like — the good, the difficult, the expensive, and the unexpected — so you can make an informed decision about whether becoming a physician is worth it for you.

🩺 Pharmacy Students & Other Nontraditional Premeds

If you’re already in another healthcare profession or pursuing another degree, starting over can feel overwhelming.

I’ll share what I learned about the MCAT, applying to medical school, interviewing, finances, balancing responsibilities outside of school, and navigating medical training as someone who didn’t take the traditional path.

🥼 Medical Students Who Want to Learn Alongside Me

I’m still learning too.

I’ll share lessons from rotations, studying, board exams, residency applications, clinical medicine, productivity, career development, and eventually residency itself.

Think of this newsletter as a running notebook from someone a few steps ahead of some readers and a few steps behind others.

The Biggest Questions About Going From PharmD → MD

If you’ve seriously considered becoming a physician after pharmacy school, you’ve probably asked yourself some version of these questions:

1. Is becoming a physician actually worth it after already becoming a pharmacist?

2. Am I too old to go back to school?

3. How do I know whether I genuinely want to practice medicine or I’m simply dissatisfied with pharmacy?

4. Is it financially insane to leave a pharmacist salary and take on medical-school debt?

5. How much income am I giving up by spending another 7–10+ years in medical training?

6. Does being a pharmacist actually make medical school easier?

7. Will medical schools view being a pharmacist as an advantage?

8. How do I explain why I want to become a physician when I’m already a healthcare professional?

9. What is medical school actually like compared with pharmacy school?

10. If I could go back and make the decision again, would I still choose PharmD → MD?

Those aren’t theoretical questions to me.

I’ve had to answer them myself.

And that’s what makes this newsletter different.

I’m not writing about what I think going from pharmacist to physician would be like.

I’m documenting what it actually is like.

Subscribe now

What You’ll Get From The PharmD to MD Scribe

Every week, I’ll take something I’ve experienced, learned, struggled with, succeeded at, or changed my mind about during this journey and turn it into something useful for you.

The framework you gave me makes an important distinction here: readers need a tangible, repeatable asset, not simply “Michael’s writing.”

So here’s what you can expect:

💊 PharmD → MD Lessons

The things I wish someone had told me before starting this journey.

I’ll break down questions like:

PharmD vs MD training

What pharmacy prepared me for

What pharmacy didn’t prepare me for

Whether medical school was what I expected

Opportunity cost

Student loans

Career satisfaction

Applying as a pharmacist

Explaining your career change

Choosing a specialty

And whether I’d do it again

🩺 Lessons From Medical Training

I’ll share what I’m learning in real time.

That means lessons from:

Medical school → clinical rotations → board exams → residency applications → Match → residency → and beyond.

The newsletter should evolve as I do.

Five years from now, you’ll be able to look back through an archive documenting almost the entire transition from pharmacist to practicing physician.

📝 Practical Guides & Resources

I don’t only want to tell stories.

I want to create resources I wish existed when I started.

Things like:

Application frameworks

Personal statement lessons

MCAT advice

Medical-school study systems

Rotation advice

Board-exam lessons

Financial breakdowns

Residency application lessons

Checklists

Templates

Mistakes to avoid

When I figure something out that could save someone else time, money, or frustration, I’ll write it down and share it.

📓 The Scribe

And finally, I’ll document the journey itself.

The wins.

The failures.

The doubts.

The things medicine teaches me.

The things pharmacy taught me that I didn’t appreciate until medical school.

And the lessons I’m learning while trying to become a great physician without losing sight of everything else that matters in life.

That’s why I call it:

The PharmD to MD Scribe.

I’m writing it down as I go.

Subscribe now

Who Am I?

I’m Michael C.
